The Opportunity

The Advanced Practice Pharmacist (APP) is responsible for providing comprehensive medication management services to improve patient outcomes across primary care and specialty practices. Working collaboratively with physicians, advanced practice providers, nurses, and care teams, the APP practices at the top of their license to optimize medication therapy, manage chronic disease states, conduct medication reviews, and support value-based care initiatives.

The APP serves as a clinical expert in medication management and provides direct patient care through collaborative practice agreements, evidence-based interventions, patient education, and care coordination. This position plays a critical role in improving quality metrics, medication adherence, patient safety, and overall population health outcomes.

What you’ll do

  • Provide comprehensive medication management services for patients with chronic and complex medical conditions.

     

  • Evaluate and optimize medication regimens in accordance with evidence-based guidelines and organizational protocols.

     

  • Initiate, modify, monitor, and discontinue medications pursuant to applicable regulations and collaborative practice agreements.

     

  • Conduct patient assessments and medication reviews to identify drug therapy problems and opportunities for improved outcomes.

     

  • Monitor therapeutic effectiveness, adverse drug reactions, medication adherence, and patient safety concerns.

     

  • Collaborate with physicians and interdisciplinary teams to develop individualized treatment plans.

     

  • Provide patient counseling and education regarding medications, disease states, treatment goals, and self-management strategies.

     

  • Document clinical interventions, patient encounters, and medication recommendations within the electronic health record (EHR).

     

  • Participate in quality improvement initiatives designed to improve clinical performance and patient outcomes.

     

  • Support population health programs focused on chronic disease management, preventive care, and medication optimization.

Who you are

  • Doctor of Pharmacy (PharmD) degree from an accredited college or school of pharmacy.

  • Current and unrestricted Pharmacist license in the applicable state.

  • Advanced Practice Pharmacist designation or eligibility in accordance with California regulations

  • Completion of a Pharmacy Practice Residency (PGY1) or equivalent clinical experience.(Preferred)

  • Minimum of two (2) years of clinical pharmacy experience in ambulatory care, primary care, managed care, or related settings.

  • Strong knowledge of chronic disease state management and evidence-based treatment guidelines.

  • Experience utilizing electronic health records and clinical documentation systems.

  • Excellent communication, collaboration, and patient education skills.

  • Ability to manage multiple priorities while maintaining high-quality patient care and service

Salary range$150,000—$170,000 USD
